1 Who We Are
Favored Healthcare Services is a Georgia-licensed home health agency based at 4297 Buford Drive, Ste. 2-B, Buford, GA 30518. We also operate the John-Florence Clinic and Lab in Yaba, Lagos, Nigeria. In this policy, "we," "us," and "our" refer to Favored Healthcare Services. This policy explains how we handle personal information collected through this website.
2 What Information We Collect
We collect information you give us directly. This includes your name, email address, phone number, mailing address, the content of messages you send us, and information you provide in forms (such as career applications, referrals, or consultation requests).
We also collect basic technical information automatically when you visit the website: your IP address, the type of device and browser you use, pages you view, and how you reached our site. This information is used to make the website work and to understand general traffic patterns.
3 How We Use Information
We use the information you give us to respond to your inquiry, schedule care or consultations, process job applications, follow up on referrals, send updates you have subscribed to receive, and to improve the service we offer. We do not use your information for purposes that are unrelated to these, and we do not make your personal information available to advertisers.
4 Legal Basis for Processing
Where privacy law requires us to identify a legal basis for handling your information, we rely on the following: your consent (for example, when you opt in to our newsletter or to analytics cookies); the steps needed to respond to your request or to take steps toward providing a service you have asked about; our legitimate interest in operating, securing, and improving the website in a way that does not override your rights; and compliance with legal obligations that apply to us. You may withdraw consent at any time where consent is the basis we rely on.
5 Cookies and Tracking
We use a small number of cookies to keep the website functioning. Essential cookies (for things like session management) are always on. Analytics cookies (which help us understand how the site is used) and marketing cookies (which help us measure outreach) are off by default and only enabled if you opt in through the cookie preferences banner.
Your cookie preferences are remembered for six months. After that, we will ask again. You can also clear cookies through your browser settings at any time.
6 Do Not Track and Global Privacy Control
Some browsers can send a "Do Not Track" signal or a Global Privacy Control (GPC) signal. Because our analytics and marketing cookies are already off by default and only run when you opt in, we do not use tracking that a Do Not Track signal would need to switch off. Where the law treats a GPC signal as a valid request to opt out of the sale or sharing of personal information, we honor it, and in any case we do not sell or share your personal information.
7 Protected Health Information (HIPAA)
We do not collect Protected Health Information (PHI) through this website. Patient names, dates of birth, diagnoses, and similar details are collected directly by phone or in person, where we can apply HIPAA-compliant safeguards. Referrals submitted online intentionally exclude PHI; we follow up by phone for those details. Any PHI we hold in the course of providing care is governed by our HIPAA Notice of Privacy Practices, not by this website policy.
9 Third-Party Services
Some parts of the website use third-party services to function: Google reCAPTCHA for bot protection on forms, Brevo for email delivery, Google Maps for the embedded map, Google Calendar for the training schedule, Tally for the client satisfaction survey, ElfSight for displaying live Google Reviews, and (when enabled) Google Analytics. These services have their own privacy policies, and information you provide directly to them is governed by those policies. This list may change from time to time as we add or remove tools.
10 Third-Party Links
Our website may link to other sites we do not operate, such as our scheduling, payment, training, or store subdomains and external resources. When you follow a link to another site, that site's own privacy policy applies. We are not responsible for the content or privacy practices of sites we do not control, and we encourage you to review their policies before providing information.
11 Email Marketing
If you subscribe to our newsletter, we use your email only to send you the updates you signed up for. Every newsletter contains an unsubscribe link, or you can email us to be removed. Unsubscribing from marketing email does not stop service-related messages you have asked us to send, such as a reply to an inquiry.
12 How Long We Keep Information
We keep information only for as long as it is needed for the purpose it was collected. As a general guide, contact-form messages are kept for up to about two years, career application materials for up to about one year (unless you ask us to remove them sooner), and newsletter subscriber records until you unsubscribe. When information is no longer needed, we delete it or remove details that identify you.
13 International Data Transfers
We operate in both the United States and Nigeria. Depending on how you reach us, information you provide may be processed in either country or by service providers located elsewhere. Where information is transferred across borders, we take steps to see that it remains protected consistent with this policy and with applicable law, including, where it applies, the Nigeria Data Protection Act 2023. If you contact us from outside the United States, you understand that your information may be processed in the United States.
14 Your Privacy Rights
You can ask us to confirm what information we hold about you, provide a copy of it, correct it, or delete it. You can also ask us to stop sending marketing email and withdraw any consent you have given. Email [email protected] with your request and we will respond within 30 days. We will not treat you differently for exercising any of these rights.
15 State Privacy Rights (California and Georgia)
If you are a California resident, you may have the right to know the categories and specific pieces of personal information we collect, to request deletion or correction, and to opt out of the sale or sharing of personal information. We do not sell or share personal information as those terms are defined under California law. You may exercise these rights by contacting us using the details below, and you may use an authorized agent to submit a request on your behalf.
Georgia does not currently have a comprehensive consumer privacy statute. We extend the core rights described in this policy, access, correction, and deletion, to Georgia residents as a matter of practice.
16 Automated Decision-Making and Profiling
We do not use your personal information to make decisions about you solely by automated means, and we do not build advertising or behavioral profiles about you. Care, hiring, and referral decisions are made by our staff.
17 Children's Privacy
This website is intended for adults. We do not knowingly collect information from children under 13. If you believe a child has provided us with personal information, please contact us so we can delete it.
18 Security
We use industry-standard technical measures (HTTPS, secure password hashing, hardened admin access, server-side validation) to protect information. No system is perfectly secure, however, and we cannot guarantee that information transmitted over the internet will never be accessed without authorization.
19 Changes to This Policy
We will post any changes to this policy on this page and update the "Last updated" date. Material changes will be highlighted at the top. Your continued use of the website after a change takes effect means you accept the updated policy.
