This notice describes how medical information about you may be used and shared, and how you can get access to this information. Please review it carefully.

Our promise to you
We keep your health information private. We are required by law to do so. This notice explains what we do with your information, what your rights are, and how to reach us if you have a concern.
How we may use and share your information
For your care. We share information with the caregivers who come to your home, so they know how to care for you. We may share it with your doctor, a hospital, or a pharmacy involved in your care.
To get paid. We share information with Medicaid or your insurance company so they will pay for your care.
To run our agency. We use your information to schedule visits, check the quality of care, train our staff, and meet the rules of the Georgia Department of Community Health.
When the law requires it. For example, if we are required to report suspected abuse or neglect, respond to a court order, or cooperate with a health department.
In an emergency. If you need emergency care, we may share what is needed to help you.
Sharing with your family
If you are there and able to tell us, we will ask you first and follow your answer. If you cannot tell us, we will share only what a family member or friend needs to know to help with your care.
If you want a family member to have their own access to your records through our family portal, you must tell us in writing. You can change your mind and take that access away at any time.
When we need your written permission
We will ask for your written permission before we:
- Use your information for marketing
- Sell your information (we do not do this)
- Use your photograph or your story publicly
- Share psychotherapy notes
- Share your information for anything not described in this notice
You can change your mind. If you give permission and later withdraw it, we will stop, except where we have already acted on it.
Your rights
You can see and get a copy of your records. Ask us. We will respond within 30 days. We may charge a reasonable fee for copying and postage.
You can ask us to correct your records. If you think something is wrong, tell us. We will respond within 60 days. If we disagree, you can write a statement and we will keep it with your records.
You can ask who we have shared your information with. We will give you a list covering the last six years. The first list each year is free.
You can ask us to limit what we share. We do not have to agree, but we will consider it. If you pay for a service yourself in full, you can tell us not to share that with your insurance, and we must agree.
You can ask us to contact you a different way. For example, at a different telephone number or address. We will accommodate reasonable requests.
You can get a paper copy of this notice at any time, even if you agreed to receive it electronically.
You can complain. See below. We will never treat you differently for complaining.
Our duties
- We are required by law to keep your health information private.
- We must give you this notice and follow what it says.
- If there is a breach of your unsecured information, we must tell you.
- We must get your permission for uses not described here.
- If we change this notice, the new version applies to information we already hold, and we will make the new notice available.
